  • Common risk factors for congestive heart failure include hypertension, coronary artery disease, diabetes, obesity, and cardiomyopathy.
  • Although brain natriuretic peptide correlates highly with symptoms of heart failure, other conditions such as advanced age, obesity, cirrhosis, and hypoxia also can increase values.
  • ACE inhibitors are recommended in most patients with CHF but must be used in high doses to show reductions in mortality.
  • Aldosterone antagonists have been shown to reduce mortality and hospital admissions in patients with NYHA class III or IV heart failure.
  • Patients with advanced heart disease can have up to 50% one-year mortality, with half of these deaths being sudden.

Recent Advances in Wound Care — June/July 2010
In the United States, wound care constitutes a large part of the care provided in any emergency department (ED). While many lacerations and traumatic injuries can be repaired easily, some require complex and time-consuming management. Patients seek treatment for wounds in EDs for several reasons: emergency physicians, with expertise in wound management, are available around the clock; many primary care offices are not equipped to treat complicated wounds; and some primary care physicians are not comfortable with complex wound closure techniques. Patients are most concerned about cosmetic outcomes, functional preservation, relief from pain, and preventing infection.
