Overview:

Twenty years after the advent of hospital case management, many organizations still struggle with model design, staffing ratios, and workloads. Indeed, it seems that every year, the newest regulation or strategy gets added to the long list of functions already performed by busy hospital case managers.

So, what is the right model for your department? What are the right staffing ratios? And how do I get the most from the staff I currently have? As a case manager, how can I go home at the end of the day feeling good about what I have done?

What roles and functions truly belong within case management? Which can be shared, and which should be managed by other members of the interdisciplinary care team?

This presentation will review the most contemporary case management roles and functions. Also included will be two of the most popular case management models, including determination of caseloads. If you are struggling with determining how to manage all the functions you are being asked to perform, this session is for you!!

Learning Objectives:
At the conclusion of this session, participants will be able to:

  • Describe the most common roles and functions of the hospital case manager.
  • Explain best practice hospital case management models.
  • Describe ways in which you can manage ever increasing workloads.
