Overview:

Transitioning patients to the next level of care is an increased challenge for case mangers. Meeting regulatory compliance, such as the important message for Medicare patients, assuring coordinated care planning with patients and families and meeting the needs of the uninsured and underinsured impact are every day demands. Both RN and Social Work case managers have significant responsibilities in assuring patients are discharged from the hospital when medical necessity is met.

Starting discharge planning upon admission does not always assure the patient will be ready for transition when the order for discharge is written. Strategies must be in place for these complex discharge planning situations, and strategies are not only the responsibility of the case manager, but also of the multidisciplinary discharge planning team.

Toni G. Cesta, Ph.D., RN, FAAN is Vice President - Patient Flow Optimization for the North Shore – Long Island Jewish Health System. Additionally, Dr. Cesta serves as Health Care Consultant and partner in Case Management Concepts, LLC, a consulting company assisting institutions in designing, implementing and evaluating case management models, new documentation systems, and other strategies for improving care and reducing cost. Dr. Cesta has presented and published extensively on case management topics. She has presented at national and international conferences and workshops, and is the author of "Nursing Case Management: From Essentials to Advanced Practice Applications" and the best selling book - "The Case Manager's Survival Guide: Winning Strategies for Clinical Practice". Dr. Cesta has also been active in the research and development of case management for 20 years. She is Consulting Editor of "Hospital Case Management Journal" and serves as editorial advisory board member of several case management journals and publications including "Strategies for Healthcare Excellence" and "The Journal of Care Management".

Beverly Cunningham, MS, RN is vice president, clinical performance improvement, Medical City Dallas Hospital, where she is responsible for Case Management, Quality, Health Information Management and Patient Access. She is nationally known for her work in the area of case management and has presented seminars and spoken at national conventions. Beverly has extensive experience in health care management. Through her consulting, she directs hospitals, physician groups and other health care facilities to redesign delivery systems focusing on case management implementation through a focus on improved clinical and financial outcomes.
