There are increased challenges in transitioning a patient from the hospital to the community, whether it is home without assistance, or home with home care or DME.

With the growing numbers of people who are not eligible for government-funded programs, hospital case managers must know how to help individuals and families navigate the healthcare system and coordinate healthcare resources.

This webinar will address how to handle transition challenges, and how case managers can use innovative resources to aid and facilitate the growing number of unfunded patients.
