This presentation will reveal a unique model of ED case management that is being utilized at Winchester Medical Center in Winchester, VA. Ms. Garrison will demonstrate a unique patient-centered approach to emergency department care management. Emphasis is placed on innovation, resourcefulness, community, and health care partnerships, and client empowerment.
