As health care reform continues to move forward, hospitals and health care providers are exploring strategies of change.   Some of these strategies must focus on how we reduce readmissions through transitional and community services.   

Join us for this 90-minute webinar on transitional and community strategies by case management experts Donna Zazworsky, RN, MS, CCM, FAAN and Dr. Toni Cesta.  They will discuss how hospitals can expand their Continuum of Care through transitional and community programs that focus on reducing readmission rates and length of stay.  These strategies include building hospital-based community programs, such as Transitional Nursing Programs and Navigator Services; as well as establishing community partnerships and programs with community-based organizations, such as home health agencies, skilled nursing facilities and many others.

4e6769f0e7442zazworsky-donna.jpgDonna Zazworsky is the Vice President for the Community Health and Continuum Care division of Carondelet Health Network, a four hospital, integrated health delivery network in Southern Arizona. Ms. Zazworsky has authored numerous articles and chapters on community case management, disease management, caring for the uninsured/underserved and advanced practice nursing.  Most recently, she was the lead editor and author of The Handbook of Diabetes Management, a resource textbook for community health providers published by Springer.
Ms. Zazworsky is an adjunct Clinical Associate Professor at the University of Arizona, College of Nursing, and Incoming President of the Southern Arizona American Diabetes Association, and Past Chair of the Communication and Publicity Committee for the American Academy of Nursing.

Cesta, Ph.D., RN, FAAN is Senior Vice President for Operational Efficiency and Capacity Management at Lutheran Medical Center in Brooklyn, New York.  Additionally, Dr. Cesta serves as Health Care Consultant and partner in Case Management Concepts, LLC, a consulting company assisting institutions in designing, implementing and evaluating case management models, new documentation systems, and other strategies for improving care and reducing cost. Dr. Cesta has presented and published extensively on case management topics. She has presented at national and international conferences and workshops, and is the author of "Nursing Case Management: From Essentials to Advanced Practice Applications" and the best selling book - "The Case Manager's Survival Guide: Winning Strategies for Clinical Practice". In 2009 Dr Cesta co-authored a new book "Case Management Core Skills" with Beverly Cunningham. Dr. Cesta has also been active in the research and development of case management for 20 years. She is Consulting Editor of "Hospital Case Management Journal" and serves as editorial advisory board member of several case management journals and publications including "Strategies for Healthcare Excellence" and "The Journal of Care Management."
