Donna Zazworsky, MS, RN is the Vice President for the Community Health and Continuum Care division of Carondelet Health Network, a four hospital, integrated health delivery network in Southern Arizona. Ms. Zazworsky has authored numerous articles and chapters on community case management, disease management, caring for the uninsured/underserved and advanced practice nursing. Most recently, she was the lead editor and author of The Handbook of Diabetes Management, a resource textbook for community health providers published by Springer. Ms. Zazworsky is an adjunct Clinical Associate Professor at the University of Arizona, College of Nursing, and Incoming President of the Southern Arizona American Diabetes Association, and Past Chair of the Communication and Publicity Committee for the American Academy of Nursing.
Toni G. Cesta, Ph.D., RN, FAAN is Senior Vice President for Operational Efficiency and Capacity Management at Lutheran Medical Center in Brooklyn, New York. Additionally, Dr. Cesta serves as Health Care Consultant and partner in Case Management Concepts, LLC, a consulting company assisting institutions in designing, implementing and evaluating case management models, new documentation systems, and other strategies for improving care and reducing cost.
