Sponsored by Case Management Advisor and Hospital Case Management
Care coordination processes are vital at every hospital. More and more staff members, such as advanced practice nurses, clinical nurse leaders, RN case managers, social work case managers, and patient navigators, all have their hand in care coordination. The effectiveness of care coordination can result in decreased LOS, decreased readmissions and an increase in patient satisfaction. Each stakeholder, with the addition of physicians, must understand their care coordination role and its relationship to length of stay (LOS).
Join us for this 90-minute webinar which is part one of the Foundations in Case Management: A Series for New and Experienced Case Managers by case management experts Dr. Toni Cesta and Bev Cunningham, RN, MS who will address strategies for effective LOS overhaul that includes both an admission and discharge time out. Also included will be the regulatory impact on care coordination and the challenges faced by complex patients, complex families and complex payer issues.
Bonus tools will be provided!
