Part 1: Case Management Across the Continuum: Seamless Processes for Transitioning and Managing Patients
During part one of the “Timely and Effective Transitions” webinar series by case management experts Dr. Toni Cesta and Bev Cunningham, RN, MS, they will discuss linking patients with the continuum. Public reporting, according to the recent Federal Registers, will focus on costs associated with the patients stay, as well as costs associated with post-acute care. Additionally, readmissions are being publicly reported. CMS has warned us that any time data, such as these are posted on the Hospital Compare website for more than a year, it can become a part of value-based purchasing. Case managers and case management leaders must be sensitive to identifying patients who contribute to high cost and/or high readmission rates. It is critical that case managers are aware of their next level of care providers so patients are not readmitted from the next level of care.
